Olmsted Falls High School 2023 Prom

All students will need to fill out this form online in order to purchase a ticket. If the guest is not an Olmsted Falls student, the guest form, emergency medical form and a photocopy of their ID must be uploaded to the google form. The Prom Letter will give more information. It is also where the guest forms are located.
